Emplify Health’s CommunityLink: Connecting Residents With Local Support Services

communitylink logo 2026 02 19

Emplify Health is spreading the word about CommunityLink, its innovative online platform designed to provide residents with easy access to essential community resources. This tool aims to empower individuals and families by connecting them to services that support health, wellness and overall quality of life.

CommunityLink serves as a comprehensive directory of local resources, including food assistance programs, housing support, mental health services, transportation options and more. By centralizing this information, Emplify Health hopes to reduce barriers to care and promote healthier communities across the regions served by Emplify Health by Bellin and Emplify Health by Gundersen.

Danielle Wnek, Community Health Improvement Manager, Population Health at Emplify Health, emphasizes the importance of CommunityLink in supporting the pillars of health.

“CommunityLink is more than just a resource directory – it’s a bridge to wellness,” Wnek said. “By providing easy access to vital services, we help individuals address critical aspects of their physical, mental and emotional well-being. This tool supports our commitment to holistic health by ensuring people have the resources they need to thrive.”

To help users navigate the platform, Emplify Health has created a detailed demonstration video available on YouTube. The video walks viewers through the features of CommunityLink, showing how to search for resources by category, location and need. The demonstration can be viewed here: CommunityLink Demo

CommunityLink is designed with user-friendliness in mind, featuring an intuitive interface accessible from any device. Whether someone is seeking immediate assistance or planning for long-term wellness, the platform offers a reliable starting point to connect with community programs and services.

CommunityLink is part of Emplify Health’s mission to improve health outcomes by addressing the broader factors that influence well-being. By integrating this resource into its population health strategy, Emplify Health continues to lead efforts in creating healthier, more resilient communities throughout Wisconsin, Iowa, Minnesota and the Upper Peninsula of Michigan.

Residents, healthcare providers and community organizations are encouraged to explore CommunityLink and share it with those who may benefit. Emplify Health remains committed to expanding and updating the platform to ensure it reflects the most current and comprehensive information available.
